EIP Student Data CollectionPlease note: The information on this page is for schools currently participating in Connecticut's Early Intervention Project (EIP). Thank you for your ongoing support and commitment to Connecticut’s Early Intervention Project (EIP). As we mark our twenty-second anniversary, your continued assistance is sought in submitting EIP Student Data that is essential for the continuation of funding for this initiative. This year, we are excited to share with you the option of completing student data using a Microsoft Excel spreadsheet designed specifically for this purpose. It is our hope that this will alleviate paperwork, be more useful to you in tracking student information, and expedite data submission. 2007-2008 Data Collection filesDownloading tip: RIGHT-CLICK on the links below and choose "Save Target As..." to download the files to your hard drive. Microsoft Excel Spreadsheet: The files include a newly revised master copy of the 2007-2008 Student Data Form (Rev. 12/06), instructions for the data collection form (to avoid common reporting errors), a transmittal/cover sheet (designed to provide school contact information should clarification of reporting be necessary) the original memo from Michelle LeBrun-Griffin dated 1-4-08, and the Excel EIP student data spreadsheet. A copy of completed Student Data Forms, accompanied by a cover sheet, should be mailed no later than June 20, 2008 to: EIP/Student Data, SERC, 25 Industrial Park Road, Middletown, CT 06457. Excel data, along with a completed cover sheet, may be transmitted to SERC via email to eipstudentdata@ctserc.org. Data files should be saved by district and school name (e.g., East Hartford, Norris).
